A 29 year old married Sikh woman, resident of Kharghar underwent abortion by surgical method- D&E- Dilatation and Evacuation as her pregnancy was reaching 11 weeks and she could not know that she is pregnant as her previous menses have been irregular.

The procedure was uneventful and she was discharged on the same day.

It has been more than 2 months that she did this abortion.

After abortion process , she got her next menses just a couple of days back.

But they were so heavy that she had to take anti-fibrinolytics medications to stop her menses.

We had even done an Ultrasound , post abortion, to check if the abortion process was complete and the report had been normal.

After abortion process generally it is seen that the next menses are irregular, they may be early, they may be late, they may be scanty, they may be heavy.
We have no control over that

A 22 year old unmarried woman, resident of Road Pali, Panvel came for consultation for unwanted pregnancy.

She is a working professional of an IT company in Kharghar, Navi Mumbai.

The pregnancy that she was carrying was of 10 weeks duration.

Though she shared that she has plans to get married to the guy, who is accompanying her and from whom she has gotten pregnant but marriage will not happen for next two years for social and financial reasons.

The man in question is also working and earning very well.

The live in relationship couple opted for abortion of this pregnancy by D&E- surgical cleaning after proper counselling.

A 31 year old woman ,resident of Nerul, Navi Mumbai,came for consultation for missed periods.
She works as a school teacher at Kharghar

Her pregnancy test came positive and as per sonography the gestation age was 5.5. weeks.

She was not desirous of keeping the pregnancy and opted for abortion tablets.

When the second dose of misoprostol is going on, and bleeding is happening, at time woman needs to rest at home.

She expressed that it will be a great difficulty to avail leave.

Office politics, ongoing election duties everything was happening at the same moment and she was quite disturbed.

Yesterday on 12.10.19, a team of doctors from Kharghar Doctor's Association met with Mr Haresh Kini, a eminent local politician and candidate for upcoming MLA election.

The agenda had been to discuss burning issues that the residents of Kharghar are facing.

One of the health services related issue is lack of Government provided health care center in this node.

The one existing Urban Health Center at sector 13, Kharghar has very rudimentary services.
It does not even have an Ultrasound machine.

For a woman desiring legal abortion either by pills or surgical D&C, has to go to a private practitioner and pay the charges as per their norms.

Safe abortion is every woman's right .

A healthy discussion was carried out .

A 34 year old resident of CBD Belapur, Navi Mumbai, came for consultation for UTI.

She shared that she is about to get re-married.

The husband is a NRI.

A 39 year old patient, resident of Taloja, Kharghar consulted us for missed periods.

She is a lecturer of Economics at a prestigious college in Pnavel, Khanda colony.

She has been married for 11 years and has a living female child of 10 years of age who has Down's syndrome.

When we did urine pregnancy test for her it came as positive but the joy of becoming pregnant was absent.

She shared that she has been living with the idea of becoming mother once again solely for the purpose of producing a child that could look after this mentally abnormal girl child of Down's syndrome when parents are no longer living.

Though the idea is far fetched, it was occupying her whole life.

For unknown reasons whenever she conceived it resulted into either missed abortion or spontaneous abortion many times.

One of the times it even turned out to be an ectopic pregnancy for which she required Laparoscopic salpingectomy.

So much physical trauma and mental disappointment sent her into mental depression and social phobia.

She stopped socializing, became irritable, on the verge of giving up her career, she required many months of counselling and psychiatric medicines.

These complicated social scenarios are very difficult to manage.

Sexual experimentation in teenage girls-Repercussions..?Outcome.....??

A young bubbly girl of 16 years, resident of Road Pali came to OPD for a Gynec consultation for irregular menses.

While consulting, she got into chit chatting.

She shared that she is a class 11 student of a reputed school from commerce stream.

She and her friends have been enjoying Garba sessions as celebrations of Navratri festival.
She also shared the latest that has been going on within her friends.

That trend is of sexual experimentation.

A couple of girls have already done that and a few more have been considering.

It was more a matter of fun and peer copying rather than any kind of emotional attachment or bonding.

What is worrisome is that this sector of society is totally oblivious to contraceptive usage, sexually transmitted diseases and risk of unwanted pregnancy.

Our education/society/parents/teachers also do not seem to be ready to look this matter in the eye and acknowledge that this is happening.

Real Women-Real Experiences- Early cervical cerclage in case of previous second trimester loss

A 34 year old women , resident of Kharghar, sector-12, came for consultation for her pregnancy.
There were couple of remarkable points in her history.

This was her second marriage and she was married for 4 years.

From previous marriage, she had a male child of 11 years of age.

In this marriage, she opted for abortion pill once as the pregnancy had occurred very early just after the marriage.

Next time when she became pregnant, it resulted into spontaneous miscarriage at 14 weeks of duration . The reason cited was short cervical length and in ultrasound it was 2.8 cm.

This time she has conceived with some difficulty and basic fertility treatment.

Now she is 7-8 weeks pregnant .

Normally cervical cerclage is advisable after 12 weeks NT , NB scan and double marker as it rules out many congenital abnormalities.

We can not take this chance as process of miscarriage may start even before first trimester is complete.

Pressure to bear a child in this marriage, her advancing age are the factors behind taking this decision of early cerclage for optimal outcome.
