REAL ISSUES (1) -   CAREER VS FAMILY

Growing urbanization has come with cost of family values.
When it comes to prioritization it is seen that career is taking precedence over family expansion.

A woman in her mid thirties who was pregnant for the first time came looking for abortion . Her husband was in early forties . The couple had been married for more than a decade.
They had postponed child bearing till now because they were both looking for settling their respective businesses.
The pregnancy that had happened was a chance one and unplanned.
They experienced momentary joy before dipping low.
They realized that with advancing age the course of pregnancy will require more monitoring .
They were unwilling to make changes to their work pattern to accommodate the newborn.
They had concerns that if they choose to have the baby, they will be at retiring age by the time the kid reaches the college age.

What we are discussing over here is the changing paradigms of the society.
Merging of roles for both genders.

Unfulfilled biological role - will it have far reaching physical and emotional impact.

It is a known medical fact that the women who have never given birth and breast fed their kids are predisposed to certain cancers.

We, the gynecologists at 'My Gynaec World' are involved in providing excellent comprehensive women care. We ponder on current society issues because we care.

It is our own mothers and sisters who make this society.

In our practice, at times we see cases of missed abortion.It means that the heart activity in the early pregnancy either did not come or the heart activity stopped after a few weeks. Many a times the patients do not want to undergo surgical procedure that is D&C as its treatment for removal of products of conception . In these case they request for oral tablets to be given. As the process of bleeding happens at privacy of home, the acceptance rate for tablets is higher.

A 32 year old married women ,resident of Kharghar,Navi Mumbai came looking for prescription of abortion pills. Her elder child was of 2 years old and she was unwilling to continue with the present pregnancy.
On examination her UPT- urine pregnancy test was positive.
Though ultra-sonography did not reveal any pregnancy . We denied giving abortion tablets and asked her to do repeat Ultrasound after a week. She agreed to our advise albeit reluctantly.
Repeat USG showed an abnormal right tubal pregnancy -ectopic pregnancy later on.
A woman with ectopic pregnancy should not be given abortion tablets as the treatment for this condition is entirely different and mainly surgical.

The discussion here is about precautions before giving abortion tablets by the Gynaecologists.
Though the Ultrasound test is not mandatory, in our opinion it should be offered to all those who can avail this facility and can afford it too.

Saving the girl child and women empowerment are talked about a lot. When we actually look into the lives of real women, we find that what we say is much farther from the truth. 
Dr. Deepa Kala - a  senior Gynaecologist practising in Seawoods, Navi Mumbai has penned down beautifully these very emotions in the form of a POEM.

                           AM   I  REALLY   FREE

Day in and Day out I hear about womens liberation thee,
But as I look around I ponder
Am I really free.
A result of their love I am,
Still they rethink if they really want me,
Just because it’s a she.
My mother sheds tears of joy on holding me in her arms,
While the absence of a heir the whole house mourns,
Her controlled dance of joy makes me think –
 Is she really free ?
Laxmi of the home they call her,
Having a say in spending money is another matter,
Annapurna of the home she is called
But she has the morsel which is left last,
She is catering to all demands
But has no one to demand from.
Her daily struggle makes me wonder,
Is she really free ?

Unequal distribution of food and fruit, a bias for games,
The drooping of faces as my brothers fare poorer than me,
Things I could not explain as I grew now make me think
Am I really free.
Prospective grooms my family screens
And I feel like I am in a scanner,
The exchange of green finally decides my life partner
And I am left thinking Am I really free.
I have a new name , A new home,
But my role remains the same,
I am told when to eat or sleep,
Choice of  job or home is made for me
Even after loosing my identity, Am I really free?
First I was in my father’s home,
Then I am in my husband’s home,
The home is incomplete without me,
But which is really my home?
These questions in my mind trouble me all through
And I continue to wonder Am I really free?

Life comes fullcircle and I am holding my daughter in my arms,
The family scene seems similar but I laugh and sing to express my joy,
Finally I have the chance to be a drop of change in the sea,
And I am going to set my princess free.

Abortion by Tablets-Real Women,Real Experiences-9

A 22 year old unmarried women, resident of Khanda Colony, Panvel  came for consultation regarding unwanted pregnancy.She was accompanied by her elder sister. They were looking for option of ' Abortion by tablets'. As the stage of pregnancy was more advanced than permissible limit of tablets, surgical method -D & E ( Dilatation & Evacuation ) was suggested.
Subsequently procedure was carried out uneventfully. 
During counselling, they shared that parents were unaware of the situation.
The boyfriend responsible was nowhere to be seen.
Though recovery in this procedure takes a few hours and patient is discharged the same day, in this particular case, they stayed in the hospital till next day.
Such that when she reaches home , no sign of weakness could be made out by the parents.

We have empathy for this young generation. They are ignorant of the health hazards & long term complications .
Non communication with parents and elders make them vulnerable to falling in wrong hands/ quacks.

Navi Mumbai is a Modern city and an advanced level education centre. Many reputed business schools, fashion institutes and medical colleges are located here along with other graduate and post graduate institutes.

Students of different parts of Maharashtra and for that matter all over India come to this city, stay in hostels and get their education. Along with the students who are residents of Navi Mumbai.The young population also include professionals who are either staying on their own, in paying guest facility or with their families.

What we are  discussing here today is the behavioural liberalisation of young generation.
We as associated Gynaeclogists of My Gynaec World In Navi Mumbai area, bring into light , that  liberal behaviour has its consequences.

We are absolutely non-judgemental about the choices that we as human beings make but these liberal choices  have some consequences on physical and mental health.

One such consequence is Unwanted Pregnancy.

As a Gynaecologist when we are clinically managing such situations, these young couples sometimes share their lives with us.

When they come for consultation they are apprehensive, anxious and afraid.
Many young people have not told their parents about the pregnancy, which increases their difficulty when their case is being managed.

There are some questions which crop up in us treating Gynaecologist's mind.
