A mother brought her daughter for consultation who was of 17 years of age. They had married her one year ago but the ceremony of ‘Gauna’ was yet to be performed. Meanwhile she was meeting up with her husband and now she was pregnant.  They were from village and belonged to poor socioeconomic strata.

Obviously the pregnancy is unwanted.

Termination of unwanted pregnancy in underage patients requires certain legal and social formalities.

 Many a private practitioners will be unwilling to take such responsibility.

There is a long waiting list in Govt. run hospitals and setup. Privacy also is an issue and patients are unwilling to opt for such facility.

In such desperate situations ,patients may fall into wrong hands and become prey of illegal practices

The heart of the problem lies in continued education and making rules simpler for the Gynaecologists and health care providers for easy and safe accessibility to abortions either by tablets or surgical D&C

A 32 year old married woman, she has taken abortion tablets one month prior. Bleeding happened normally and the process of abortion was complete.
The next month, again the periods were overdue. 
On checking , the results showed that she is pregnant again.
Repeat procedure of taking abortion pills needed to be carried out .

The point here is , that in the time period after taking abortion pills, it is very important to use alternative methods of contraception/family planning( precautions ) so that you are not pregnant again.
In the next month you may plan for regular contraceptives like oral pills, Copper-T, injectable contraceptive or may even adopt permanent method of sterilisation.
In case of female it may be laparoscopic tubal ligation.
In case of male it is vasectomy

Repeated abortions by tablets is a health hazard
It may lead to anaemia, chronic ill health, vaginal infections and white discharge and future infertility.

A 26 years old unmarried woman,  underwent abortion by tablets a couple of weeks back. 
She is working and living in a different town than her parents. 
A complication of excessive bleeding arose and subsequently she also required a DNC 
Amongst all of that,  the parents were unaware of her struggle with medical condition. 
She being scared to tell them. 
Medically speaking, she was managed.
What about our social responsibility.

Probably the influence is Western. It requires further research into what is leading to changed sexual and social behavior of today's youth. 
Why unwanted pregnancies are on a rise. 
Pregnancy rate is increasing in unmarried couples, but our social structure is yet to change. 
These pregnancies are not resulting in live births. 
Naturally demand for abortions and the resultant complications are on rise. 
Low health, subsequent infertility, guilt, shame,  woman's desire to go back to virginal state as now she is getting married to a different man.
All of these clinical conditions we are seeing,  are probably 10 times more than just a decade back. 
Education, awareness and easy and safe access to medical facilities is the need of the hour

A 27 years old unmarried woman walks into the OPD with complaints of irregular menses.
She underwent certain investigation and it turned out that she is not pregnant
She gives history of being in relationshiph and she has taken tablets for abortion  multiple times over past few years.
She now suffers from recurrent vaginal infection
One thing that worries her more is that she is rejecting physical intimacy at all levels from the guy , with whom she is going to get married.
She has physical and mental trauma.How long will that last , remains to be seen.

Will abortion pills work in a pregnancy which has yet to show in uterus

Question

I want to do abortion by pills, my urine pregnancy test is positive but sonography is not showing any pregnancy in uterus. Will abortion pills work for me?

Answer

The best way to deal with this situation will be to wait for a couple of days, repeat the sonography after that.
Once the pregnancy is shown in the uterus, the chances are high that abortion pills will do their work properly.

Pregnancy showing heartbeat, Should I go for abortion by pills or DNC

My pregnancy is showing heartbeat and duration is around 6 weeks and 1 day.Should I opt for abortion by pills or go for traditional DNC

Answer
Legally speaking, you can be prescribed pills for abortion as the duration of pregnancy is within the limit.
However it is our experience, that the rate of incomplete abortion and subsequent requirement of DNC for its management is more in such patients who opt to take tablets.
Tablets work best if the growing embryo has not yet developed heart activity.
It is best to have frank discussion with your Gynaecologist and then make an informed decision.
