                     First case of unmarried woman wanting to continue early pregnancy

-A 24 year old young unmarried working woman , resident of Mankhurd, Mumbai, came for consultation with positive pregnancy test.

-As is our routine practice, we asked her, whether she would like to continue or discontinue with her pregnancy.

-Her answer came as , primarily, she would like to continue with her pregnancy but still she would go back to her home and discuss with her parents before taking a decision.

-Her partner was with her and he too agreed with her thought process.

-In 17 years of my practice, this was the first time that I saw unmarried couple taking path of continuation.Rest all of them have opted for termination of pregnancy due to financial, social, emotional and so many other factors.

-A couple of days passed, she again came back to us but this time to opt for abortion pills.

-Her parents strongly condemned her life choices.

-The point her is , that we are progressing towards sexual behaviour liberation and pre- marital sex. We want to enjoy sex but not take responsibility of life that gets created as its outcome .

-The change in outlook , though it happened in the couple but family and social environment was not supportive

A 26 year old woman, resident of Nerul,Navi Mumbai, a dentist by profession, has been married for 6 months.

She was not looking to become pregnant, but her pregnancy test came weak positive after missing periods for 3 days.

She was to travel to another city for a function in family, hence was desirous of removing pregnancy as early as possible.

In the meantime she also started with spotting per vaginum of some blood.

When an Trans- Vaginal ultrasound was done- it showed - no G-sac ( Gestational Sac ). 

A conclusion of Chemical Pregnancy was made. It is a known clinical entity- wherein sperm and ova after meeting- start to develop a zygote but for so many reasons- mainly genetic- fail to produce a healthy pregnancy.

Nature's way of removing it is - a bit delayed menses.

Many a times if Urine Pregnancy Test ( UPT ) is not performed, diagnosis of this condition is completely missed.

It is a common phenomenon in IVF ( In Vitro Fertilization ) cycles as , both patient as well as the treating Gynecologist are eager to know the result of their trial and will perform Beta HCG test even before the expected date of menses.

It will come a bit high but then menses will resume.

The clinical significance is that these reactions are not to be regarded as pregnancies , turning into early abortion. 

If that approach is taken , a lot of unnecessary heartache, headache and investigations will be avoided. 

The key of management is- DO NOTHING

A 26 years old unmarried, working woman, resident of Kandivali, came for consultation for history of irregular and delayed menses. She is engaged to be married and was looking for regularization of menses.She was sexually active .

As a part of protocol of first line investigation of missed periods , we did urine pregnancy test (UPT) & it came out positive.

She showed surprise at this finding as she has been menstruating albeit irregularly.

An obstetrics ultrasound was performed and it revealed pregnancy of 15-16 weeks duration.

Now, at this gestation age , abortion tablets are out of question.

Even a procedure of D & E ( Dilatation and Evacuation ) also popularly known as D&C can not be performed as a method of termination of pregnancy.

Only thing remaining is induction of mini labor that is legal and safe process of second trimester (Duration of Pregnancy Between 12-20 Weeks of Gestation ) MTP ( Medical Termination of Pregnancy )

As this is a normal pregnancy , obviously uterus is in no mood to go in labor yet.

We have excellent Uterotonic ( making uterus contract to expel pregnancy ) drugs, and we made use of them.

It took a day of medications, lots of IV and oral medication and fetus got expelled. 

After a day of delay , under Short General Anesthesia ( SGA) a  CC-check curettage was also performed to remove adherent pieces of placenta.

She was discharged from the hospital uneventfully.

The impact on physical health , mental health , emotional health, financial health has been tremendous..

Simple use of regular contraception like daily oral contraceptive pill (OCP) or regular use of condoms would have prevented so many of heartache and tremendous amount of pain

A 29 year old female, resident of Sector-20, Kharghar, Navi Mumbai came for consultation of early pregnancy.

She has had LSCS delivery 1.3 years back and subsequently has had a spontaneous abortion a couple of months back.

She was in dilemma about continuation of this pregnancy.

Spacing between children is important, as well as allowing some time to elapse before next pregnancy is planned in case of previous Caesarean delivery.

Advancing age i.e. childbearing after prime age , also comes with its own set of complications.

A 24 year old woman, resident of Kamothe- Sector 11, came to OPD for consultation for Abortion tablets, 

-She had a living female child of 2.5 years of age by normal delivery. ( G-2,P-1,L-1)

-She had missed her menses and her UPT- Urine Pregnancy Test was positive.

-She has been complaining of on & off menstrual like lower abdomen cramps, which on some moments were severe.

-But she did not start with menses.

-On examination- her lower abdomen was slightly distended.

- Pulse rate was 92/min and BP- 100/70 mm Hg

- As a routine test and precaution before giving abortion pills , Trans-Vaginal ultra sound was performed.

-The findings suggested intra-abdominal free fluid amounting to 700-800 ml

-There was G-Sac ( Gestational sac ) like structure near left ovary which was surrounded by a mass of tissue , which seemed to be attached to left ovary.

-Diagnosis of Ovarian ruptured ectopic pregnancy was suspected as there was a breach in the outer lining of ovary too and adnexal mass was lying just close to that, along with echogenic mass lesion in the ovary (Trophoblastic Tissue).

- She was immediately posted for Laparoscopy by Dr Santosh Jaybhaye , Om Sai Hospital, Kamothe.

- On putting the scope, the first finding that we noted was Haemoperitoneum ( Blood in peritoneal cavity)

- Upon suctioning and cleaning it up, it was then possible to locate ruptured Left ovarian ruptured ectopic pregnancy.

- Mass lesion was resected ( surgically cut ) and Haemostasis ( There is no longer bleeding ) was achieved.

- She required one unit of packed cells blood transfusion intra-operatively and her recovery post operatively was uneventful.

-Following routine safety protocols, timely intervention and availability of blood products saved the life of the woman

A 27 year old woman, resident of sector 11, Kharghar, came to OPD for consultation.

She has one living male child of 16 months of age by Cesarean delivery.

She is a teacher by profession and just now has joined back her job after long maternity leave.

She missed her menses and urine pregnancy test came positive.

We have been taking care of her from her first pregnancy days.

By nature she is of very soft temperament and very emotional, very gentle.

Even though the pregnancy is very early , she is still facing the dilemma of letting go of a potential life.

In situations like this there is no standard counseling that can be done and depends on individual's response.
